Comprehensive Assessment

All new clients undergo our full comprehensive assessment. It is recommended that all appointments be done on separate days to optimize the opportunity for you to talk openly and honestly with each professional and learn incrementally through each interaction. A comprehensive treatment plan will be developed throughout this process and shared with you as the assessment progresses. By the final assessment session, recommendations will be discussed, which will provide guidance regarding ongoing services at HUM and referrals to external resources as needed.

Part 1-Nursing Assessment

1.5 hour appointment with a Registered Nurse

During this appointment, presenting issues and concerns will be addressed and you will complete a bio-psycho-social-spiritual questionnaire. Completion of the comprehensive questionnaire will address previous and current health issues, family history, etc. in order for us to have some general background prior to you coming to HUM. This allows us to provide appropriate recommendations and care.

Part 2-Therapist Assessment

1 hour appointment with a Registered Psychologist or Clinical Social Worker + Optional Psychological testing

During this appointment, the therapist will build on the information that was gathered in the first session, further address current issues and concerns, discuss significant relationships, mental health diagnoses (e.g., depression, anxiety) and chronic pain issues, if applicable. During this appointment, the therapist will ask about your treatment goals and provide suggestions for treatment planning
This assessment is holistic and focuses on the individual’s bio-psycho-social-spiritual health.

Individuals have the option to complete a psychological test as part of their second appointment. The test takes 30-50 minutes to complete and measures mood and anxiety disorders, personality disorders, substance use, social support, self-harm, perception of self and relationships. Results of the test will be discussed at a follow up appointment once the test has been scored.

The comprehensive assessment is available to clients who are located out of town and can be done through Skype.

Optional-Medical Assessment

30-60 minute appointment with a medical doctor

If requested or, determined through the assessment it would be recommended, you will meet with one of the medical doctors on our team. During this appointment, your specific needs will be reviewed and actions recommended as required. Complementary and integrative medicine areas can also be reviewed depending on the individual’s needs and interests.

Please be advised that Alberta Health Care (AHC)  only covers medical appointments if you attend your appointment.

We have a 48 hour cancellation policy for all appointments. If you are unable to attend your appointment and do not provide advanced notice, you will be charged for the full cost of the appointment, including Medical appointments


Fee for Comprehensive Assessment :    $600

  • $400 for the Assessment (Parts 1 and 2)
  • Alberta Health Care covers optional sessions with physicians
  • $200 deposit kept on file for any late cancellations, no shows and unpaid balances (this can be requested back at anytime)
