All new clients undergo our full comprehensive assessment. It is recommended that all 3 appointments be done on separate days to optimize the opportunity for you to talk openly and honestly with each professional and learn incrementally through each interaction. A comprehensive treatment plan will be developed throughout this process and shared with you as the assessment progresses. By the third and final assessment session, recommendations will be discussed, which will provide guidance regarding ongoing services at HUM and referrals to external resources as needed.
Part 1-Nursing Assessment
1.5 hour appointment with a Registered Nurse
During this appointment, presenting issues and concerns will be addressed and completion of a bio-psycho-social-spiritual questionnaire. Completion of our comprehensive questionnaire will address previous and current health issues, family history etc in order for us to have some general background prior to you coming to HUM.
Part 2-Therapist Assessment
1 hour appointment with a Registered Psychologist + Psychological testing
During this appointment, the therapist will build on the information that was gathered in the first session, further address current issues and concerns, discuss significant relationships, mental health diagnoses (e.g., depression, anxiety) and chronic pain issues, if applicable. During this appointment, the therapist will also begin to outline treatment goals and provide suggestions for treatment planning
This assessment is holistic and focuses on the individual’s bio-psycho-social-spiritual health.
Individuals have the option to complete a psychological test as part of their second appointment. The test takes 30-50 minutes to complete and measures mood and anxiety disorders, personality disorders, substance use, social support, self-harm, perception of self and relationships. Results of the test will be discussed at a follow up appointment once the test has been scored.
Part 3-Medical Assessment
1 hour appointment with a medical doctor
During this appointment, the medical doctor will review part 1 and 2 of the assessment and address current issues and concerns. Medications will be reviewed and diagnoses clarified. An individualized treatment plan and recommendations will be outlined with you tailored to your specific needs and goals, in context of what may be needed biologically, psychologically, socially and spiritually for maximizing the individual’s health potential. Family issues and community resources will also be explored.
Complementary and integrative medicine areas would also be reviewed depending on the individual’s needs and interests.
Please be advised that Alberta Health Care (AHC) only covers the Medical assessment and follow up Medical appointments if you attend your appointment. We have a 48 hour cancellation policy for all appointments. If you are unable to attend your appointment and do not provide advanced notice, you will be charged for the full cost of the appointment, including Medical appointments
Fee for 3 part Comprehensive Assessment : $600
- $400 for the Assessment (Parts 1 and 2) as Alberta Health Care covers Part 3
- $200 deposit kept on file for any late cancellations, no shows and unpaid balances (this can be requested back at anytime)
