Privacy Policy



Health Upwardly Mobile Inc. (HUM) is committed to the privacy and security of all information as well as the confidentiality of all personal information. HUM complies with the requirements of managing privacy and confidentiality in accordance with the provisions of the Personal Information Protection Act (PIPA) and Health Information Act (HIA). HUM also adheres to the provisions of the College of Physicians and Surgeons of Alberta. We have a designated Privacy Officer on staff and HUM is responsible for the personal information and your privacy with regard to the collection, use and disclosure of your information as well as the availability, accuracy and notification/communication around collected personal information.

HUM maintains its records in a secure environment. The protection of information against unauthorized access, use and disclosure is maintained at the highest level. HUM will not sell your personal information for any purpose whatsoever.

Collection of personal information

At Health Upwardly Mobile Inc. (HUM), your information is collected to ensure that a comprehensive assessment, treatment, health services and education can be provided. As a client of HUM we collect social, demographic, health and a medical history as well as contact and payment information. All information is collected with your consent.

If you are an employee of an employer who has hired HUM to provide you with an assessment, treatment or health services, we may collect personal information as well as information about your job. HUM may be asked to share aggregated statistics with your employer but your personal information will not be identified from these statistics. We are committed to the privacy of your information and have safeguards in place to ensure complete privacy and anonymity.

Limiting Use, Disclosure, and Retention

Personal information will not be used or disclosed for purposes other than those for which it was collected, except with the consent of the individual or as required by law. Personal information will be retained only as long as necessary for the fulfillment of those purposes, unless required by law to retain such information for a longer period of time.

Personal information will be stored in a secure environment and accessed only by authorized HUM associates as retained by HUM. Personal information will be retained and/or destroyed in accordance with HUM policies and applicable laws.

Access to personal information

For clients using one or more of the services offered at HUM (eg. counselling, therapy, occupational health services), access to information is strictly controlled. Personal and health information is disclosed to HUM associates only to perform their assigned duties and ensure high quality care. Only the least amount of health information necessary to carry out the intended purpose is collected, used and disclosed.

With your written consent, we will forward information to your family physician or other third party service provider to ensure appropriate ongoing monitoring and long-term follow up. Your information will not be forwarded without your consent.

Individual Access

If requested, a HUM associate shall inform you of the existence, use, and disclosure of your personal information and you will be given access to that information. An individual shall be able to challenge the accuracy and completeness of the information and have it amended as appropriate.

Note: HUM may deny access to certain personal information it holds about an individual if the information is prohibitively costly to provide, if it contains references to other individuals, if it cannot be disclosed for legal, security or commercial proprietary reasons, or if it is subject to solicitor-client or litigation privilege. HUM will advise the individual of the reason for denying the access request.
